A COMPLAINT WAS FILED WITH THE HEALTH DEPARTMENT CLAIMING THAT THIS FACILITY HAS LOST EMPLOYEES CERTIFIED AS FOOD SAFETY MANAGERS AND, THEREFORE, DOES NOT HAVE ENOUGH CERTIFIED FOOD SAFETY MANAGERS TO COVER ALL SHIFTS.
UPON ARRIVAL, I INFORMED PAUL MANTAS OF THE COMPLAINT AND HE CONFIRMED THAT TWO INDIVIDUALS, ZACHARY MULLADY AND SALAVOR MARQUEZ JR., ARE NO LONGER EMPLOYED, AND FOR SHORT PERIOD OF TIME, THE RESTAURANT DID NOT HAVE ENOUGH FOOD SAFETY MANAGERS FOR COVER ALL SHIFTS. HOWEVER, MR. MANTAS STATED THAT HE HAD HIRED TWO EMPLOYEES WHO ARE CERTIFED AS FOOD SAFETY MANAGERS, INCLUDING ALEX MAZARAKOS AND NICHOLAS MAZARAKOS (SEE RISK ASSESSMENT FOR CERTIFICATION DETAILS). IN ADDITION, HE IS TRAINING ADDITIONAL STAFF TO BECOME CERTIFIED AS FOOD SAFETY MANAGERS.
